Crucible Performance Waiver

By using the facilities, equipment, or services at Crucible Performance, you agree to the following:

1. Assumption of Risk

   I understand that training, exercise, and the use of fitness equipment involve risks, including injury, illness, permanent disability, or death. I choose to participate voluntarily and accept full responsibility for my own safety and the safety of any guest I bring.

2. Release of Liability

   I agree that Crucible Performance, its owners, staff, directors, insurers, and partners are not responsible for any injury, illness (including COVID), damage, or loss that occurs in, on, or around the facility, or while using any equipment or participating in any activity. This applies even if the injury or loss is caused in whole or in part by the negligence of Crucible Performance.

3. Full Waiver of Claims

   I release Crucible Performance from any and all current or future claims, demands, or causes of action related to my participation or my guest’s participation. This includes both known and unknown claims.

4. Indemnification

   I agree to protect, defend, and hold Crucible Performance harmless if any claim is made by me, my guest, or any third party due to injuries or damages connected to our participation.

5. Personal Responsibility

   I agree to use the equipment properly, follow all posted rules, and conduct myself in a safe and reasonable manner at all times. I understand that Crucible Performance does not provide constant supervision.

6. Property Disclaimer

   I understand that Crucible Performance is not responsible for lost, stolen, or damaged personal property.

By signing or submitting this waiver, I acknowledge that I have read and fully understand the terms above, and I willingly agree to them.
